2. One original and one photocopy of the agreement the foreign doctor signed with the inviting and employing organization; or one original and one photocopy of letter of guarantee signed by the foreign doctor with the inviting and employing organization. 

1. One original (for examination only) and one photocopy of notarized diploma of degree of the foreign doctor. 

2. One original (for examination only) and one photocopy of notarized license for practice of medicine or qualification certificate for practice of medicine in foreign countries.

1. The first page of the application form for foreign doctors' short-term practice of medicine in Beijing shall be filled in both Chinese and English. The rest should be filled in Chinese, and all blanks shall be filled in. 

2. The agreement signed by the foreign doctor with the inviting and employing organization and the letter of guarantee signed by the foreign doctor and the inviting and employing organization shall include: purpose, specific project, location and conditions, including the conditions required to finish the project, for example, the technological strength, housing, equipment and medicine of the inviting organization; term of agreement; and undertaking of responsibilities.

3. When accepting scanned electronic materials and materials submitted online, the form shall be affixed with an electronic seal.
