
The materials shall be complete and meet statutory requirements: 1. The establishment of a medical institution shall comply with the medical institution establishment plan and the basic standards for medical institutions. 2. Applicants are ineligible to apply for establishing a medical institution if one of the following conditions applies: (1) The applicant is an entity that cannot independently assume civil liability. (2) The applicant is an individual currently serving a sentence or lacking full capacity for civil conduct. (3) The applicant is a medical professional who is currently employed at a medical institution, has retired due to illness, or is on unpaid leave. (4) The applicant is a medical professional who has been involved in medical incidents classified as level 2 or higher within the past five years. (5) The applicant is a medical professional whose license to practise medicine has been revoked due to violations of relevant laws, regulations, or rules. (6) The applicant is the legal representative or the main person in charge of a medical institution with its Practice License of Medical Institution being revoked or suspended. Individuals falling under any of the circumstances outlined in items (2), (3), (4), (5), or (6) above are ineligible to serve as the legal representative or the main person in charge of a medical institution.

V. If two or more legal entities or other organizations jointly apply for establishing a medical institution, or if two or more individuals jointly apply for establishing a medical institution, they shall submit an Agreement on Application for Establishing a Medical Institution.
VI. For the establishment of a for-profit medical institution, the applicant shall submit a business license.
VII. Project Construction Plan (signed by the legal representative of the joint venture and cooperative parties)
VIII. Respective registration certificates of the joint venture and cooperative parties, credit documents for all joint venture and cooperative parties), Resident Identity Card of the People's Republic of China of the applicant's legal representative
IX. If the application involves state-owned assets investment, confirmation documents from the state-owned assets management authority shall be submitted.
