1. Proof of identity
A. The household register and ID cards of the person placing out a child for adoption and the adoptee
B. Valid passport of the adopter. The adoption registration authority will photocopy the original, archive the photocopy and return the original to the adopter. (The original is for inspection only and a photocopy is also required.)
2. An original adoption registration certificate (The original shall be archived.)
3. Written agreement on the termination of the adoptive relationship (self-provided)
The adopter and the person placing out a child for adoption shall jointly sign the agreement at the adoption registration authority. If the adoptive parents and the adult adopted child agree to terminate the adoptive relationship, involvement of the person placing out the child for adoption is not required.
4. Two-inch recent half-length bareheaded photos of the adopter, the person placing out the child for adoption and the adoptee, respectively
Where a social welfare institution is the party placing out a child for adoption, such institution is not required to submit a photo. The photos shall be colorized photos
General requirements for application materials
1. If materials are incomplete or invalid, staff at the reception window will not accept the application.
2. Photocopies shall be legible and identical to the originals.
3. The above proof materials are valid for 6 months.

1. If the domestic residence of the adoptee is cancelled and the adoptee obtains a foreign nationality, the domestic civil affairs authority cannot handle the termination of the adoptive relationship.
2. Where the adoptee is underage, the adopter and the person placing out the child for adoption shall terminate the adoptive relationship on a voluntary basis. If the adoptee is aged 10 or more, such termination is subject to his or her consent. If the adoptee has reached adulthood, such termination is not subject to the involvement of the person placing out the child for adoption. The application materials submitted are complete and valid.
3. Adoption registration shall be handled in the Chinese mainland;
4. The adopter and the adoptee still maintain the adoptive relationship to date;
5. Where the adoptee is underage, both the adopter and the person placing out the child for adoption shall have full capacity for civil conduct. Where the adoptee has reached adulthood, both the adopter and the adoptee shall have full capacity for civil conduct;
6. An agreement on the termination of the adoptive relationship has been reached.
