1. Application form for permit (academic exchange) for foreign doctors to practice medicine for short term in Beijing (one copy);

The first page of the form shall be filled in both Chinese and English, and other pages in Chinese. No blank cells.

2. Two recent photos: 1 to 2 inches, bareheaded, front-facing;

3. The original permit for foreign doctors to practice medicine for short term (one original);

4. A medical license or permit for clinical privileges of foreign doctors (valid, officially stamped by a medical institution, Chinese translation required for materials in foreign languages);

5. Agreement or warranty of liability signed between foreign doctors and the inviting or employing institutions (one copy);

6. Practice License of Medical Institution of the employing institutions (one copy of the duplicate);

7. Letter of authorization signed by foreign doctors for such registration purposes.
