1. An original copy of the valid passport of the adopter (the original is for inspection only);

2. An original copy of notice of adoption in China;

3. An orginal copy of the marriage certificate of the adopter and the biological father or mother of the adoptee (the original for inspection only);

Registration of marriages abroad must be notarized and certified, and translated into Chinese in the same format in full text.

4. A original copy of the written opinion of the adoptee's biological father or mother and a notarial certificate certifying that such parent has agreed to the adoption (self-provided);

5. An original copy of the household register and ID card of the person placing out the child for adoption and the adoptee (the original is for inspection only);

6. The biological father or mother shall submit an original copy of the agreement signed with the local family planning commission on not violating the family planning provisions (the original is for inspection only);

7. An original copy of the written medical certificates of the adoptee's birth (the original is for inspection only);

8. If the adoptee is aged 10 or more, an original copy of the statement on the adoptee's agreement to the adoption shall be submitted;

9. Photos.

- An individual photo of the adopter, the person placing out the child for adoption, and adoptee (3.5 cm × 4.9 cm, bareheaded, color photo against a single-colored background, self-provided);

- A group photo of the adopter and adoptee (3.5 cm × 5.3 cm, color photo against a single-colored background, self-provided).

General requirements for application materials:

- If materials are incomplete or invalid, staff at the reception window will not accept the application;

- Photocopies shall be legible and identical to the originals;

- The above proof materials are valid for 6 months.
