1. The original of the registration application form for the establishment of singing and dancing entertainment venues in Beijing which will be operated by Sino-foreign joint ventures or Sino-foreign cooperative enterprises (not required for online application), or the original of the application form for the establishment of game and recreation entertainment venues in Beijing which will be operated by Sino-foreign joint ventures or Sino-foreign cooperative enterprises is needed. (Both application forms shall be stamped with an official seal and the content shall be authentic.)

2. Original written statement of no circumstances specified in the Regulations, affixed with an official seal. A written statement issued by the relevant agencies of the country or region where the foreign investor is located (or the embassy or consulate in China and relevant stationed agency) after verification. This statement is legally valid and is not beholden to circumstances stipulated in articles 4, 5 and 52 of the Regulations.

3. The original legitimacy document certifying that the site can be used as an entertainment venue.

(1) Property ownership certificate. Lease documents must also be submitted for leased premises and attached with the internal structure plan. The plan of a cabaret entertainment venue must delineate the location of boxes and separate rooms. The plan of a games and recreation entertainment venue must delineate the business areas designated for games and recreation as well as the type and amount of games and recreation equipment.

(2) Certificate of fire safety inspection for the place of public gathering before it is put into use and open for business.

(3) In accordance with the Regulations on Safe Management of Hazardous Chemicals, written consent from the local industry authority must be submitted when applying for the establishment of an entertainment venue located adjacent to a disputed chemical warehouse.

(4) Approval documents issued by the environmental protection administration.

4. The original of the power of attorney is needed. (This document is required only when an individual other than the legal representative acts as an agent to submit the application, and the original of the power of attorney shall be stamped with an official seal, with a photocopy of the agent's identity document pasted on it.)
