1. The original copy of the application for the establishment of a medical institution is needed.

2. The original copy of the feasibility study report is needed. (The report shall include the name and basic information of the applicant institution, as well as the name, age, resume, and ID number of the applicant; it shall also cover the health status, prevalence of diseases among the regional population, and relevant morbidity rates of some diseases; it shall include medical resource distribution and an analysis of medical service demands in the region; it shall indicate the name, premise location, functions, tasks, and service radius of the medical institution to be built; it shall cover the service modes, time, medical subjects of diagnosis and treatment, and the number of beds of the medical institution to be built; it shall contain the organization and staff member arrangement of the medical institution to be built; it shall contain the arrangement of devices and equipment for the medical institution to be built; it shall indicate the relationship and influence between the medical institution to be built and other existing medical institutions within its service radius; it shall contain wastewater, sewage, and fecal sludge management and treatment plan of the medical institution to be built; it shall cover the communication, power supply, water supply and sewerage, and fire-fighting facilities of the medical institution to be built; it shall cover source of funds, investment modes, total investment, and registered capital (capital); investment budget of the medical institution to be built; finally, a cost-benefit analysis (within five years) of the medical institution to be built shall also be provided.)

3. The original copy of the premise selection report and floor plans of the architecture design is needed. (The document shall include the basis for site selection, the environment, and public facilities in the area of the premise; it shall cover the distance between the premise and the childcare institutions, primary and secondary schools, and food production and operation organizations nearby; it shall contain land area and floor space.); The original copy of the building orientation diagram is needed; the original copy of the use certificate of the premise (for verification only) is needed.

4. The original copy of the statement confirming that the entity or the applicant applying for establishment meets the application requirements (The statement shall verify that the legal entity or individual is not listed in Article 12 of the Detailed Rules for the Implementation of the Regulations on the Administration of Medical Institutions), or the original copy of the certificate of the legal entity (for verification only) is needed; The original copy of the credit certificate (the entity or applicant applying for establishment) is needed. 

5. The original agreement for the establishment of a medical institution submitted by two or more legal persons or organizations jointly applying for the establishment of a medical institution or the original agreement for the establishment of a medical institution submitted by two or more partners applying for the establishment of a medical institution is needed. 

6. The original copy of the business license of for-profit medical institutions is needed.

7. A project construction plan (signed by the legal representatives of the partners of joint venture and cooperation) is needed.

8. The original copy of the registration certificates (partners of joint venture and cooperation), the original copy of the credit certificate (partners of joint venture and cooperation), and the legal representative's original Resident Identity Card of the People's Republic of China (the legal representative of the entity applying for the establishment of a medical institution) (for verification only) are needed.

9. For the investment involving state-owned assets, the original confirmation document issued by the state-owned asset supervision and administration department shall be submitted.

All the aforementioned materials shall be stamped with the official seal of the entity. If the materials are submitted online with an official electronic seal, there is no need to submit written materials on-site. For foreign investors of joint ventures or cooperation, the legal person's registration certificate, legal representative's identity certificate, and bank credit certificate shall be notarized by a notarial office in the country where the foreign investor of the joint venture or cooperation or the wholly foreign-owned enterprise is located or certified by the country's embassy (consulate) in China. Any materials submitted in a foreign language shall be translated by a company with translation qualifications in China or translated and notarized by a Chinese notarial office.
