1. The original Application Form for the Alteration of Commercial Performance in Beijing (For Foreign-related as well as Hong Kong, Macao and Taiwan-related Performances), which is stamped with an official seal.

(1) The application form shall use a unified template available on the official website; (2) It shall be stamped with the official seal of the applicant; (3) The performance name noted in the application form shall be consistent with that in the system and other paper documents; (4) The performers listed in the application form shall be consistent with those in the system and other paper documents; (5) The performance date noted in the application form shall be consistent with that in the system and other paper documents; (6) The performance venue noted in the application form is consistent with that in the system and other paper documents; (7) The application shall be submitted at least 21 days prior to the performance date; (8) The performance name shall not contain the words "Chinese", "China", "national", or "international".

2. The original copy of the approval document issued by a governmental department in charge

(Please submit the original copy of the approval document while obtaining the new approval document);

3. The original letter of approval from the new venue after the alteration of venue (It shall be stamped with an official seal; The agreement signed by the performance organizer and the performance venue provider shall also be attached, including, but not limited to, the name of the performance organizer and performance date);

4. The original copy of the description on the new performance venue.

One copy of each of the following documents stamped with official seals:

(1)An Entertainment Business License, Record Certificate of Operating Body of Performance Venue, or Certificate of Qualification for Fire Safety Inspection of Public Gathering Places Before Use and Operation. (2) For large-scale commercial performances to be held on temporary stages and grandstands, safety and security work plans, fire suppression and emergency evacuation plans, and other safety, and the approval documents related to security and fire prevention and obtained in accordance with the law, shall be submitted. (3) Where live performance activities are provided to the public in real time through the Internet, a Network Culture Business License (business scope includes "performances and programs"), live streaming agreements and emergency plans of the network platforms concerned shall be submitted.

5. The original copy of the power of attorney

(It shall be stamped with an official seal; the power of attorney is required for a non-legal representative to submit the application concerned, with the photocopy of his-her ID card stamped);

3. Procedure

(1) Application Processing: four working days for application/reception and one working day for processing

(2) Verification and Decision-making: five working days for decision-making

(3) Issuance and Delivery: four working days for certificate making and one working day for issuance
